Job Summary: Professional Care Match, LLC. is seeking compassionate, dedicated individuals to join our team as In-Home Caregivers. As a CNA, HHA, PCA – In Home Caregiver, you will provide essential healthcare and personal support to clients in their own homes. This is a full-time, hourly position in Holyoke, Massachusetts, requiring a commitment to providing exceptional care to our clients, who may include elderly, disabled, or chronically ill individuals. Responsibilities:

• Provide daily assistance with activities of daily living, such as bathing, dressing, grooming, and medication management
• Perform household tasks, including meal preparation, light housekeeping, and laundry
• Monitor and report changes in client’s physical, mental, and emotional health to the care team
• Promote a safe and comfortable environment for our clients
• Engage in meaningful activities and provide companionship to our clients
• Adhere to all care plans and follow proper infection control and safety procedures
• Maintain accurate and timely documentation of services provided

Requirements:

• Current certification as a CNA, HHA, or PCA in the state of Massachusetts
• Experience working with elderly, disabled, or chronically ill individuals preferred
• Strong communication and interpersonal skills
• Ability to follow care plans and provide compassionate, person-centered care
• Reliable transportation and a valid driver’s license
• Must pass a background check and drug screen
